Our doctors care about being gentle and effective podiatrists. From an ingrown toenail to major surgery, you will be treated as if you were family. We’ve included this page to help make your first appointment to Town Square Family Foot Care as easy and comfortable as possible. There’s a list of what to bring along and instructions for our Internet patient portal for you to complete your registration and save time at check-in.

Activate your Patient Portal
When you schedule your first appointment or if it’s been a while since your last visit, you’ll receive a “Welcome to Our Portal” email. Click on the “Activate Account” button in that email. Enter your date of birth and last name. Then set your password and login.
Be sure to complete all sections under the “My Health” section prior to your appointment to save time checking-in.
Once you have an active portal account, you can come back anytime to check results, request refills, see and pay your statement, and send us secure messages.

Download the App
When your portal has been set-up, you can then download an app for your mobile phone or tablet to access our portal easily and conveniently. There are iOS and Android versions available.

Troubles?
The initial email expires after 72 hours for routine security. If that happens, select “Forgot Password” and you’ll still be able to create a new account. If you’d like assistance, call us and we’d be happy to reset your account.
Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. For details of our billing policy, please click here. If you have any questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company. To pay your bill, click the button below.
